I would like to take this opportunity to welcome all of our new patients to MDC-Atlanta.

We are excited that you have decided to give MDC-Atlanta an opportunity to form a partnership with you in managing your quality, safe, effective, and efficient health care, while bringing convenient medical services directly to you, and saving you money.

As we form a mutually respectful relationship, upholding patient's dignity, and maintaining patient's privacy with utmost consideration, I would like to suggest the following to our new patients:

     1. Take your medication as prescribed by your physician.  If you notice any adverse side-effect by taking your medication, immediately call your physician and explain to him/her your symptoms

     2. Do your part to uphold healthy habits, i.e. sticking to a sensible diet/nutrition, and doing light exercise, as well as home monitoring your blood sugar and blood pressure

     3. Notify the doctor with any new or worsening symptom(s) or change(s) in your health condition

     4. Inform your doctor when you have been evaluated by a different doctor who may not be familiar with your primary care doctor, or if you have been recently admitted to the hospital and treated by a hospitalist (a hospital-based physician practitioner).

     5.  Call your physician at MDC-Atlanta to schedule follow-up APPOINTMENTS/VISITS-  (that is YOUR responsibility as a member of this partnership with OUR PRACTICE)
